Private Duty RN Case Manager

Job Description Job Description Summary:
The Private Duty RN Case Manager is a Registered Nurse which directs and supervises all personnel involved in the client care within a specific geographic area. Responsible for the total plan of care and services of clients and the allocation of agency resources. Assists with the monitoring of renewable requirements for all field staff.
Essential Job Functions/Responsibilities:
A.Makes initial client evaluation and assessment visits. Consult with client, family members and office staff in establishing, reviewing and updating the plan of care and services as necessary.
B.May review new case referrals. Reviews available client information related to the case, including need for disciplines that may be required to meet the client's home care and service needs. Communicates client information to the Clinical Manager and Client Services Manager
C.Consults with Clinical Manager and Director of Operations with any issues or problems concerning clients regarding client care and agency services and works to ensure appropriate coordination of services as needed.
D.Assists Clinical Manager in the supervision of home health aides, certified nurse's aides, nurses and/or other ancillary service personnel as applicable.
E.Participates in the nurse on-call system of the agency as appropriate and assigned.
F.Participates in staff development activities as assigned and according to agency policy and procedure.
G.Participates in the evaluation for the agency nursing program to ensure that care and services provided are adequate, appropriate, effective, and efficient and provides input to the Clinical Manager, the Director of Operations or appropriate corporate leadership if any opportunities for improvement are identified and/or resources identified are needed to improve client care and services.
H.Initiates and conducts regularly scheduled Care Team Conferences for clients and ensures documentation of meeting minutes is reflected in the client's record and communicated to the team per the Care Planning and Coordination Policy.
I.Conducts phone follow-up with clients per agency protocols and/or as the client condition may require.
J.Completes assessments, re-assessments and client visits and provides related supervision in a timely manner.
K.Reviews records to assure that all records are in compliance with agency policies and procedures. Ensures that all items are in place according to agency practices to ensure appropriate billing occurs.
L.Follows all privacy policies and maintains the confidentiality of protected healthcare information (PHI) at all times.
Position Qualifications:
1.Licensed Registered Nurse in the state served by the agency.
2.A minimum of one (1) year of experience as Registered Nurse along with experience in the field of private duty, home or community health is preferred.
3.A minimum of one (1) year supervisory experience as an RN in a health care setting.
4.Possesses and maintains current CPR certification.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
