General Description: ? ?Schedule:?Monday through Friday Full-Time Days
The PRP - Case Manager is responsible for coordinating program services and other services recommended by the Service Team to individuals recovering from mental illness. The Case Manager works with individuals, the Service Team and outside service providers to ensure the services are in place for consumers to develop the skills necessary for independence and success. Reports to the Psychiatric Rehabilitation Program Director.

Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions:
A Bachelors Degree plus two (2) years experience in mental health/psychiatric rehabilitation is required or an AA degree with at least three (3) years experience in mental health/psychiatric rehabilitation may be considered. Also at least one year of supervisory experience is preferred. A clean driving record and background is also required. Leadership qualities and the ability to work as a part of a team are necessary. Ability to respect individual choice in the recovery process and a commitment to psychiatric recovery is required. Also, this position requires excellent verbal and written communication skills and the ability to respect the choices and limitations of consumers.

Summary of Essential Functions, Responsibilities and Duties
Coordinate services with the Service Team
Coordinate program services (Residential, Psychiatric Rehabilitation Program, mental health services and somatic needs)
Coordinate all somatic physician, therapy and psychiatrist appointments
Obtain authorizations for Rehabilitation Services.
Responsible for monthly & bi-yearly paperwork for consumers as assigned.
Submit annual documentation updates
Complete and submit residential intensity and frequency forms initially and as changes occur.
Communicate effectively with the Intake Coordinator, Residential Supervisor and Program Director to ensure linkage of resources and services
Ensure income and eligibility for entitlements on a regular basis
Provide off site services and review weekly off-site service tickets
Responsibility for specialized services and needs such as MTA and ID cards
Daily review of communication book and respond to needed requests
Review and determine individual's leave requests
Engage in crisis management and participate in emergency on-call rotation.
Perform other duties as requested by supervisor
