4 Quick Apply Full-time 8 hours ago Full Job Description Nurse Care Manager - 30 HOURS PER WEEK with BENEFITS The Nurse Care Manager is responsible for coordinating care for individuals.
The nurse has responsibility for ensuring the implementation of the treatment care plan in partnership with the individual, family, Psychiatric Rehabilitation Program (PRP) staff and others as requested.
The nurse conducts health promotion and education, provides referrals and offers additional supports as needed.
Required Education, Knowledge, Skills and Abilities Must be a RN and possess a Maryland license.
A good driving record is required.
Skill in offering education options and developing wellness and prevention strategies is a plus.
The ability to work as a part of a team is essential.
Must possess the ability to respect individual choice and provide person centered planning.
Strong documentation skills are essential.
Must possess the aptitude to use data tools to track individual care and medical conditions.
Summary of Essential Functions, Responsibilities and Duties:
Develops wellness and prevention initiatives.
Conducts monthly individual and population based services for all Health Home enrollees.
Facilitates health education groups Provide other necessary health related training needs for individuals, family and staff.
Training on topics such as medical diseases, treatments and medications.
Consults with PRP staff about identified health conditions Works with the team to ensure communication with medical providers & hospitals for admission/discharge.
Provides medication reconciliation and ensures proper follow up care post discharge from hospitalizations Communicates with other healthcare providers as needed to coordinate care Provide services which may include medical screenings (i.
e.
blood pressure, diabetes and weight checks).
Coordinate with team to monitor and schedule regular physicals, gynecological appointments, mammograms and other somatic appointments and behavioral health appointments for individuals Tracks required assessments and screenings Available to other staff for consultation Monitors health information technology (HIT) tools & reports, pharmacy data and hospital encounter alerts.
Monitors & reports performance measures and outcomes.
Provides documentation as required for services provided.
Performs other duties as assigned by supervisor Quick Apply.
Estimated Salary: $20 to $28 per hour based on qualifications.
